Feedback Loops

Users are telling you something's wrong. The information takes three weeks and four handoffs to reach the person who could fix it.

Feedback loops are systematic mechanisms for capturing signals about system performance and feeding them back to inform adjustments. You identify the signal, shorten the path, and connect it to someone with the authority to respond.

A four-stage cycle of labelled nodes - action, observe, reflect, and adjust - arranged as a closed loop with arrows returning to action, each node carrying a one-line caption. A banner reads "feedback becomes input only when the system returns to action."
